Reinventing healthcare one patient at a time

Ellis Primary Care - Nott Street, Schenectady

Ellis Primary Care at Nott Street, Schenectady is conveniently located in the Medical Arts Building adjacent to Ellis Hospital. We have earned national recognition as a Patient-Centered Medical Home practice for the high quality, patient-focused, coordinated care we provide.

A Focus on You

Our patients are at the center of the care we provide, and we encourage our patients to be an active partner in their health care. We strive to communicate effectively with you, and ensure your care is well coordinated. A patient’s care, for example, may lead in several different directions over the course of a lifetime - to the hospital, a specialist such as a cardiologist or neurologist, a mental health professional, a social service agency, or may involve home care. Whatever your health care needs, you can count on your primary care physician to guide and support you every step of the way.

Our Services include:

  • Regular check-ups/physicals
  • Sick visits
  • Immunizations
  • Education (diet, exercise, smoking cessation)
  • Counseling
  • Diagnosis & treatment of illnesses and diseases
  • Pediatric care
  • Home Care

We see patients of all ages and are currently welcoming new patients.

Referrals & Hospital Care

Our patients have access to Ellis Medicine's experts and its many programs, services and facilities - a tremendous resource you can count on.

Should our patients require more specialized care or treatment, we will consult with or refer you to the appropriate medical expert or link you to a social service agency that can help. Should you require hospital care, our Hospitalists will admit you to Ellis Hospital, where our care team will tend to you during your stay.
