Referrals and evaluations for care

Short term rehabilitation or sub acute care patients must be referred by a physician. All candidates are screened for appropriateness for admission into the sub acute care program.

For admission into Ellis Residential & Rehabilitation Center’s long term care program, a completed admission application and a Patient Review Instrument (PRI) assessment form must be submitted for evaluation.

Accepted payment sources include private payment, Medicare, Medicaid and participating insurance programs. Prior authorization may be required per managed care contracts.
