Diabetes Education
Diabetes Day by Day is an outpatient self-management education program designed to help you to learn to live well with diabetes. Our program is facilitated by Certified Diabetes Care & Education Specialists and is nationally recognized by the American Diabetes Association as a program of excellence. This program offers participants the option to receive one-to-one instruction or take part in group sessions.
Our Diabetes Day by Day Program is available at the following location:

Getting Started
The first step of our program is a one on one meeting with a Certified Diabetes Care & Education Specialist. During the appointment, an assessment of your learning needs will be completed, and together you form a diabetes education and training plan specific to you!
What You Will Learn
Topics that are included with diabetes self-management education (but are not limited to):
- What is Diabetes?
- Blood glucose monitoring
- Training on glucometers, continuous glucose monitors (CGMs), and insulin pumps
- Complications related to diabetes
- Nutrition, meal planning and weight loss
- Role of physical activity
- Stress management
Our program is designd to help people with diabetes maintain the motivation to keep up with the hard work of taking care of themselves. Once you have completed the program, a follow-up visit with your Diabetes Care & Education Specialist will be scheduled to help keep you on track with your goals.
Group Sessions
Prefer to learn with others? Group sessions are available at various times. The sessions are held in a classroom, and facilitated by one of our Diabetes Care and Education Specialists. Group sessions will have no more than 8 participants. Each session lasts about two hours. The program consists of a series of three sessions all with various topics of diabetes care and management:
- Monitoring and Management
- Move More Eat Right
- Putting It All Together
Participants are encouraged to bring supportive family and friends that can assist with their diabetes care. Please contact our main office for upcoming class dates and times.
Fee
Diabetes Education is covered by most insurance companies. Check with your insurance plan if you have any questions. If cost is an issue, please call us for assistance at 518.347.5421.
